I agree that crate training is the best option. You also should get your carpets cleaned. Dogs can smell where they have gone to the bathroom for months after and they will soil in the same area. You can rent one of those cleaning machines at the market and do it yourself for a lot less money.

You will also want to limit the area that she is able to be in. For instance, only allow her in the living room...If you are in there, she is allowed. The main reason you want to keep her in the same room with you is so you can catch her in the act if she starts going. Clap your hands loudly, say no, then take her outside to her spot and tell her go potty. If she goes, treat her and praise her a lot. Trust me, she will get the point fast.

I would also go back to the basic potty training. A few minutes after she eats or drinks, make her go out. If she does her business, give her a treat and praise like crazy. If she doesn't, take her in and make her go out and try in another 10-15 minutes.

DO NOT rub your dogs nose in her messes if she has an accident. So many people do that and it only scares and confuses dogs. You do not want to punish her. Make sure you reward her positive behavior. Dogs respond much better to that.
References :
I have potty trained a lot of dogs

Put her into a kennel to sleep. She will not soil her sleeping space. Let her out the first thing. It is not cruel to place her there for sleeping. She may even prefer it. Leave the kennel out and open during the day. Giving her a schedule rather than waiting for her to "tell" you might work better.
References :
You could try crate training, you could try keeping her on a lead while in the house so that she isn't out of your sight, or you could try not waiting for her to ask you to go out and just take her out every hour or so.
