What do you do when the puppy pees and your too late to catch him, as in you find it 5 minutes later. What do I do?

I can't bring him outside and show him the appropriate place to pee because he doesn't have to pee anymore, AND by then he's forgotten about the whole incident in the house anyway.

I'm not having a ...
I'm not having a problem at night. My dog is GREAT in her crate, she would never pee in there, but I don't want her to live her life in there. She whines to get out during the day, but if I give her any freedom she pees in the house. I just let her in from outside and on her way to her crate she pees on the floor! I'm getting so frustrated.
