Start by taking your dog out ten minutes after the food . If he doesn't do anything continue taking him out at ten minutes intervals until he performs.this will help u to figure out how soon after he needs to pee and poop Some dogs circle, whine or sniff Know the signal so hurry your dog outside when u see them Paper training is also best for the people who lives in condos or high rises without access to a yard .Place a paper in the area where you want your dog to go. when he shows the sign take him to the paper,if he moves take him to the paper again When he performs praise him.Use sponge to capture some of urine smell next time he smells the urine and be prompted to go again.
References :
A book on dogs training

This works well

When shes done eating take her out to pee
when shes done playing take her out to pee
when she wakes up take her out to pee

and when she pees praise her big time.

Usually when puppies are done eating, sleeping or playing they always have to go potty. So you need to be watchful of what shes doing.

Smaking your dog isnt good. If she pees in the house show her the pee and firmly say "No bad dog" they will get that thats a no no and stop.

I do agree with Sarah K that your probly missing the sign that shes telling you she needs to go. One of our dogs just sits in front of you.
References :
has 4 dogs

Well I know you shouldn't do this but it worked on my dog...

When the dog goes in the house take its nose and put it in the urine or etc... and tell it 'Bad Dog' AND say over and over 'NO'. while gently smacking the nose... THEN take it by its collar and take it outside... AND tell it 'Here is where you do that'. I did that for about a week and it finally worked...

AND also, your dog might be telling you that s/he has to go but you are just missing the sign... Keep your eyes open for anything... It could be as simple as going to the back door or even walking/running in circles <-- (that's what my dog did).

Hope this helps... Good Luck!
References :
A dog owner for many years...
