crate her, don't let her play alone with the pad,

pick her up once she comes out of the Crate put her on the pad,say go pee,if she tries to get off keep putting her on till she pee's
say good girl, then play with her

if she is left in an area with a wee pad and bed and water, she is bored, she will tear up the wee pad, once she pee's Play with her poop her out,try the wee pad again she must of drank water during play, put her back in the crate..

they sell wee pads frames that the pads stay in place,or tape the pad down...

I have 5 week old puppies and they are doing the wee pad great, just won't be able to train them outside its so cold in Ontario canada

puppy may poo 15 to 20 min after eating, some will poop before, every breed or individual is different, you have to know when she needs to go,that's what potty training is all about...
References :
I don't know if you are intersted in this but my chihuahua is trained to use a litter box when the weather is to yucky for him. It is not that difficult to do and they actually make puppy litter (cat litter works fine also)
Or....you can keep your puppy leashed when she is outside of her kennel. Do not let her get any further then 6 feet away from you then she will not be able to sneak off and go potty. You can catch her quickly and take her outside. When she goes potty outside praise her. This is truely the easiest way to correct this proble. If you can not have her leashed with you then put her into her kennel (not for long periods of time though) . If she is not kennel trained then I would get this done right away. Do not let her have endless supply of food and water. Only let her have it at certain times of the day will also help to controll when she needs to get. Be persistant and you will win the puppy potty battle.
References :
Animal rescue for 10+ years and this system works great for dogs of all ages. I just housebroke a 10 year old dog for the 1st time in her life with this system.
